ANSWER:

As of October 2018, ORIGIN NeuroCare program will be available in CENTRUM ORIGIN OTWOCK, the first ORIGIN Polska facility in Poland. The program will offer neurological rehabilitation.
As of May 2019, ORIGIN NeuroCare and ORIGIN OrthoCare programs will be available in CENTRUM ORIGIN KRAKÓW. The program will offer neuro and orthopaedic rehabilitation.
Both facilities will offer inpatient and outpatient services.
Subsequent ORIGIN CENTRES are already being planned, and will follow soon.

ANSWER:

As of October 2018, ORIGIN MemoryCare program will be available in CENTRUM ORIGIN OTWOCK, the first ORIGIN Polska facility in Poland. The program will offer care to people with dementia, including Alzheimer’s disease.
More ORIGIN senior care programs will be available in ORIGIN CENTRES in the near future. They will include ORIGIN SeniorCare, long-term care programs for seniors with moderate and severe dependency, and ORIGIN Apartments - serviced residences (serviced apartments) for more functionally independent individuals.  

ANSWER:

During the Diagnostic Stay program, a team of ORIGIN specialists will conduct a multiday assessment in a designated training apartment. The report will be issued after 2-7 days of tests and observations and will include a full health and functional assessment, a list of risks and recommendations for further action. 
We offer free phone consultations and short respite stays at preferential rates for caretakers of patients who are undergoing diagnostic testing in ORIGIN CENTRE in Otwock and qualify for home care.
We offer early Alzheimer's disease detection using techniques involving biomarkers and genetic mutations in our Walk-in Clinic 

ANSWER:

The price varies depending on accommodation and required rehabilitation programs, as they demand different degrees of medical /nursing staff and physiotherapist involvement.
The final cost, which may be the same as the preliminary estimation based on documentation and recommendations of the referring physician, will be determined following the diagnostic stay in the ORIGIN CENTER. A report, prepared by a team of ORIGIN specialists, will include an assessment of patient’s overall condition and rehabilitation strategy recommendations.

ANSWER:

In CENTRUM ORIGIN OTWOCK, we offer dementia care for people in advanced stages of disease, as well as palliative and hospice care.  
The prices vary depending on stage of disease, accommodation, and supplementary services that include professional procedures and tests not provided in ORIGIN facilities (X-rays, dental services, etc.).
The final price will be determined following the Diagnostic Stay, based on a thorough assessment of patient’s condition and care strategy recommendation issued by a team of ORIGIN specialists.

The first ORIGIN serviced apartments are expected on the market in 2019.
They will be available for sale and for rent with one month’s notice.
Monthly fees will cover a wide range of therapeutic and activating programs based on the Canadian model, nutritional program, maintenance services and 24-hour access to medical and nursing ORIGIN staff.

ANSWER:

We are fully aware that loved ones’ visits bring joy to our residents’ lives. Therefore, families and friends are always welcome in ORIGIN CENTRES. All visitors are required to sign in and out at our 24-hour concierge service desk.

In ORIGIN CENTRES we provide 24-hour clinical supervision. Medical care is available on all weekdays. Medical consultation via phone is available at all times.

ANSWER:

All rooms in rehabilitation clusters are fully furnished.
All long-term care rooms and serviced apartments are equipped with special electric adjustable beds for the elderly. We also allow personal furniture.
In MemoryCare rooms we encourage bringing in small favourite personal furnishings and decorations, such as arm chairs, coffee tables, desks, pictures, framed photographs, a wall clock, etc.

ANSWER:

Our general policy allows patients to bring in small animals and keep them in private rooms and apartments. 
Residents will be required to sign and acknowledge the applicable terms and conditions.  

ANSWER:

We have a strict non-smoking policy in ORIGIN CENTRES regarding both tobacco products and e-cigarettes. The ban applies to residents, our staff, and all visitors.

With the exception of serviced apartments, the consumption of any alcohol products in ORIGIN CENTRES is prohibited. 

ANSWER:

Close elations with family and friends are very important to our residents. Independent residents and those who need minor assistance can come and go as they please. However, for security reasons, they need to inform us if they plan to stay out overnight.
For Memory Care patients with dementia, special procedure must be observed. Detailed information can be found in our pamphlet available to the families of our residents.

All ORIGIN CENTRES maintain close contact with local parishes. Spiritual support is available upon request.

ANSWER:

CENTRUM ORIGIN OTWOCK offers a broad spectrum of supplementary services, including a beauty parlour, hairdresser, personal care, personal rehabilitation programs, facultative trips, etc.

ANSWER:

Toiletries such as sponge, liquid soap, shampoo, toothbrush, toothpaste, hairbrush, comb, razor, shaving cream/foam, moisturizing body lotion, etc.
Cotton clothing suitable for the patient’s condition and appropriate to the season, including regular socks, white socks for rehabilitation and therapeutic sessions, light sport footwear tied with laces or Velcro.
